Menlo Aquatics Center Waiver

Those with current memberships do not need this waiver. It is already part of your membership. This is for daily patrons. 


Upon entering Menlo Aquatic Center you agree to abide by the rules and regulations listed at the facility and on our website at http://perkasieborough.org/menlo-aquatics-center. Our goal is to provide an enjoyable visit for all patrons. Failure to comply with our rules and regulations will result in immediate removal from the premises with no re-entry or refund.


Please read below: 
I acknowledge that I have read this waiver of liability and fully understand these terms. I agree to accept the risk of any pool use and further agree to not hold Menlo Aquatics Center employees, Perkasie Borough and its employees, or any instructors conducting classes in which I have enrolled, liable for any and all claims, suits, losses or related cause of action for personal injuries or damages that arise out of my participation. I also agree to pay for any and all damage caused to the facility by me or my actions.

I understand that this is my responsibility to consult with a physician prior to regarding my participation in swimming and water activities. I represent and warrant that I am physically fit and I have no medical condition that would interfere with my safety and/or the safety of others. I understand that I am responsible for monitoring my own condition throughout swimming and water activities and should any unusual symptoms occur, I will cease my participation and notify the instructor of lifeguard of the symptoms. 

By completing this registration I certify that I am acting as head of household in agreeing to this liability waiver on behalf of myself, family members, and minors under my supervision.

CONDITIONS OF SALE




  1. All sales of memberships and program registration fees are final. NO REFUNDS OR EXCHANGES.


  2. Membership passes are non transferable. If a pass is used by a person other than the member to whom it was issued, it will be confiscated and membership forfeited, with no refund.


  3. Purchaser acknowledges all information to be true and correct. (false resident information will result in forfeiture of membership with no refund)


  4. Children under 14 years of age must be accompanied by and under the direct supervision of an adult 16 years or older. (limit of three children per caretaker)


  5. Admission to the pool requires membership pass or daily pass. Photos are required for memberships, valid ID must be shown to purchase a daily pass.


  6. Menlo Aquatic Center will close at 4:00 pm on occasion to accommodate Pennridge Gators meets.


  7. Menlo Aquatic Center management reserves the right to close any or all pools, slides and features at any time.


  8. Signs are posted at the Menlo Aquatics Center regarding events, changes to schedules, etc. This information will also be posted on the Recreation webpage: www.PerkasieRec.com and also on facebook.


  9. Replacement charge for pool passes - $6.00 and replacement charge for swim bands - $1.00.




 
LIABILITY WAIVER
In consideration for being permitted access to Perkasie Borough Menlo Aquatics Center, the undersigned agrees on behalf of themselves, their family members and any minors under their supervision:




  1. To make use of Menlo Aquatics Center with full knowledge that such use could result in potential injury or personal property damage.


  2. To assume all risks and responsibilities associated with any injuries or personal property damage suffered in conjunction with use of Menlo Aquatics Center.


  3. To indemnify and hold harmless Menlo Aquatics Center and Perkasie Borough.


  4. That Perkasie Borough staff and Menlo Aquatics staff have the right to enforce rules of conduct and may remove guests from the premises for failure to comply with these rules. Guests are not entitled to receive a refund after such removal.


  5. Members agree to provide, if requested, proof of age.


  6. Entrance into Menlo Aquatics Center signifies an understanding and abidance of the posted rules and regulations.




 HOUSEHOLD ACCURACY




  • Proof of residence and ID’s of all family members must be shown before memberships are issued. Signing this document is considered agreement of Pool waivers and rules. Mailed applications will no longer be accepted.


  • Residents are classified by those who have electric service with the Borough of Perkasie. To purchase any resident category membership, you may be asked to submit a Perkasie Borough Electric Bill.


  • A family includes any adults and dependent children (age 18 and younger) residing in one household year round
